ABOUT THE ROLE
Reporting to the Manager - Specialized Geriatric Services the Nurse Practitioner (NP) provides optimal and efficient care and support to older adults (geriatrics) care partners and communities. The NP works within the College of Nurses of Ontario Nurse Practitioner scope of practice and works in accordance with SJCG policies and procedures. Primary responsibilities of the NP include conducting assessments providing a broad range of therapeutic interventions facilitating access to health services and devising treatment plans all focused on the client reaching optimal health.

WHAT YOU BRING
Education/Experience:
- Master's degree in nursing and successful completion of a recognized Nurse Practitioner Program – Adult
- Registered (or eligible for registration) in the Extended Class College of Nurses of Ontario
- Minimum of three years' primary care experience preferably providing health care services to the geriatric population
- Minimum of three year's mental health experience with the knowledge of the shared care model
- Advanced knowledge and understanding of best practices related to dementia delirium mental health and its effect on the person and family members/care partners
- Strong knowledge of mental health nursing theory/practice principles interventions medications and symptoms
- Knowledge and experience with various legislation including the Mental Health Act Health Care Consent Act and other relevant legislation
Skills/Abilities:
- Advanced clinical experience skills and judgement in geriatric medicine and mental health
- Superior ability to communicate effectively and concisely both orally and in writing and through a variety of means
- Strong knowledge of community and regional services
- Ability to function and contribute as an effective team member including providing mentoring education and coaching within the team
- Ability to work independently with the capacity to case review and consult with physicians
- Strong leadership skills and demonstrated ability to organize prioritize and multitask efficiently
- Excellent computer presentation and educational skills
- Strong interpersonal skills to work with diverse client groups with varying levels of comprehension and language capability
- Effective listening observation and facilitation skills to effectively assess the client and provide appropriate treatment and consultation
- Ability to use independent judgment and maintain confidentiality of records actions and information
Assets:
- CNA Certificate in Geriatric Nursing
- Membership in professional association e.g. NPAO
Conditions of Employment:
- This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) OR Criminal Record & Judicial Matters Check (CRJMC) where indicated, to ensure the absence of relevant criminal convictions
- Maintain certifications
- Ensure appropriate Professional Liability Protection is available according to the requirements of the CNO
- Will be required to travel to various work locations and provide own reliable transportation
