Full-Time Direct Support Professional - Homeshare

Compensation: $25.78-28.43 hourly

Inclusion West Niagara is seeking a dedicated and skilled Homeshare Direct Support Professional to support individuals participating in our Homeshare Program. This role plays an important part in ensuring individuals receive consistent, respectful, and high-quality supports in their home and community environments. 

The successful candidate will work collaboratively with individuals, Homeshare families, Respite providers, Passport, Day Program staff and internal team members. This position requires strong organizational skills, sound judgment, and the ability to work both independently and as part of a supportive team. 

Nature of the Position 

The Homeshare Support Worker provides a variety of personal, practical, and administrative supports aligned with individual goals and agency values. While the role includes some coordination and documentation responsibilities, the support worker will primarily provide hands-on support. Flexibility, reliability, and a person-centred approach are essential. 

Hours of Work: Regular hours are Monday to Friday, 8:00 a.m. to 4:00 p.m; however, flexibility in scheduling is required and you may need to work an occasional weekend or evening to support the needs of the program. 

Key Responsibilities 

Person-Centred & Family Supports 

  • Provide ongoing support and follow-up to individuals and Homeshare families 
  • Maintain regular communication and check-ins, increasing contact when concerns or changes arise 
  • Support individuals during medical appointments 
  • Provide personal care support on an as-needed basis 
  • Assist individuals with personal finances as required 
  • Act as a liaison with ODSP and other external service providers 

Program Administration & Documentation 

  • Collect, process, and maintain monthly Homeshare documentation 
  • Update and maintain individual fact sheets, goals, written procedures, and ISP-related materials 
  • Prepare and submit monthly Passport documentation and required forms 
  • Complete detailed documentation including appointment reports, contacts notes and incident reports as required 
  • Maintain organized, accurate records in accordance with agency standards 

Health, Safety & Medication Supports 

  • Review MAR sheets, document changes as required, monthly medication reordering 
  • Support Homeshare providers and Respite staff in medication and documentation procedures 
  • Conduct safety inspections as required 

Collaboration & Team Engagement 

  • Work collaboratively with Homeshare Providers, Respite Providers, Passport Workers, and CPS Day Program staff 
  • Provide guidance and practical problem-solving support related to daily living skills and behavioural strategies 
  • Participate in meetings and program planning 
  • Support program continuity and coverage if required 

Qualifications 

  • The candidate(s) must have a relevant 2-year Community College Diploma, related University Degree or equivalent experience in the field.
  • Police clearance with vulnerable sector screening.
  • A valid G driver's license and insurable by the agency’s insurance company.
  • Demonstrated problem-solving skills and ability to respond to individual needs and crisis situations in a calm, productive manner.
  • Well-developed organizational and communication skills are essential.
  • Valid Non-Violent Crisis Intervention (CPI)
  • Valid Standard First Aid & Level C CPR

Skills & Attributes

  • Proficient computer skills with experience using our Agency Platforms as well as Microsoft Word and Excel 
  • Strong ability to work independently and collaboratively 
  • Well-developed problem-solving and communication skills 
  • Excellent documentation, record-keeping, and report-writing abilities 
  • High level of organization and attention to detail
